- Signed & completed membership form – One application per new or renewing member. Click one of the items below to download/view the membership form.
- Click –> New Membership Application
- Click –> Renewal Membership Application
- One of the following items listed below (a to d) which shows proof of Open AMA Membership valid from date of application through or later.
- Copy of AMA card.
- Duplicate AMA card provided on your AMA renewal paperwork.
- Copy of payment receipt. This receipt must show AMA expiration date.
- Copy of membership verification from the AMA website found here.
NO Park Pilot Program Membership Accepted!
- A paper copy of your valid FAA registration card. Must not be expired at the time of renewal. Login Here to print your FAA UAS card.
- Check, Money Order, Cashier’s Check. Made payable to PVMAC. It is recommended that you do not send cash through the mail.
- (new members only) Picture (head shot-Full face in Color)-This will be printed on your membership Card. If digital, email to membership@pvmac.com. JPG only please.
